PURPOSE AND SCOPE:
Promotes quality patient care, ongoing customer support, and vascular access case management for the FMS patient population through the implementation of an integrated, multi-disciplinary vascular access pilot program, managed through the Medical Office, that assesses, documents, monitors, and trends vascular access data, identifies vascular access problems, and coordinates timely referrals for possible vascular access intervention, which may lead to reduced catheter exposure time, creation and preservation of arteriovenous fistula (AVF), and improved patient outcomes.

DUTIES / ACTIVITIES :
CUSTOMER SERVICE:
Responsible for driving the FMCNA and FMS culture through values and customer service standards. Accountable for outstanding customer service to all external and internal customers. Develops and maintains effective relationships through effective and timely communication. Takes initiative and action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner. PRINCIPAL RESPONSIBILITIES AND DUTIES Collaborates with the clinical staff, physicians, and other members of the patient’s healthcare team to develop an individualized and comprehensive vascular access plan of care, which includes AVF maturation, treatment options for stenosis, surgical or interventional treatments for aneurysm, second AVF or next access when current access fails. Ensures the coordination and integration of services as planned in the vascular access plan of care as needed to ensure high quality continuity of services and cost effective service delivery. Performs routine vascular access monitoring and surveillance on all FMS patients, including physical exam, assessing clinical signs and symptoms, and monitoring access flow measurements. Monitors status of mature or failing dialysis accesses on clinical visits, makes referrals to surgeons and interventionalists for evaluation, and communicates with both surgeon and patient as appropriate. Accompanies patients to Fresenius Vascular Access Centers as needed for care coordination.

  • Observes vascular access procedures as needed to relay pertinent patient-related observations such as cannulation issues to the treating physician.
  • Returns to the dialysis facility for the next patient hemodialysis session to relay the patient information to the dialysis facility staff members.
Collaborates with the facility staff to create and implement a patient’s self cannulation plan of care and required educational and competency to achieve self cannulation skills. Identifies patient knowledge deficits regarding vascular access care and provides the appropriate education, support and materials to facilitate informed decision making and understanding. Engages third party translation services as indicated to carry out patient education and care planning. Analyzes facility access rates and access flow results to identify trends and opportunities for vascular access process improvement. Manages and facilitates the application of vascular access algorithms. Provides education, feedback and assistance to facility staff regarding use of vascular access algorithms and interpretation of access flow measurement results. Reviews all vascular accesses and identifies any outliers (based on chart review, physical exam or dialysis treatment related data) for priority physician consultation. Utilizes critical analytical skills to perform an individual case review of the patient’s medical history, lab results, vascular access intervention reports and hemodialysis treatment data in the electronic medical record (Proton, eCube Clinicals, AMI, and the Access Center EMR). Helps the dialysis facility staff and nephrologist to understand surgical reports, intervention reports, and/or films pre and post interventions (x-ray images, surgical drawing, procedure descriptions and ultrasound results). Serves as an educational resource to FMS facility staff. Participates in vascular access workshops to provide ongoing and supplemental training to direct patient care staff and physicians. Communicates effectively with patients, FMS facility staff, and physicians regarding patient vascular access issues to ensure that all issues are addressed and resolved. Reviews and analyzes facility-specific and patient-specific vascular access reports. Provides status reports to Medical Office on a monthly basis. Evaluates patient outcomes to determine if the vascular access program is effective and suggests recommendations for improvement. Assists with training of new vascular access case managers. Collaborates with vascular access case management team to develop, modify, and improve workflow processes within the team. Participates in project-based work, assigned by the Medical Office and/or the FVC Vascular Access Specialist. Other duties as assigned.

Qualifications PHYSICAL DEMANDS AND WORKING CONDITIONS: The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Day to day work includes desk and personal computer work and interaction with patients, facility staff and physicians. The position requires travel between assigned facilities and various locations within the community. Travel to Regional, Business Unit and Corporate meetings may be required. Travel to Fresenius Vascular Care access centers may be required. The work environment is characteristic of a health care facility with air temperature control and moderate noise levels. May be exposed to infectious and contagious diseases/materials. EDUCATION RN required with active, unrestricted nursing licensure in practicing state(s). BSN preferred. EXPERIENCE AND REQUIRED SKILLS Two years of hemodialysis nursing experience required. One year medical-surgical nursing experience preferred. One year acute (ICU, CCU, acute dialysis) experience preferred. Nephrology Nursing Certification preferred. Excellent verbal and written communication skills. Demonstrated ability to function as part of a team. Must be self-motivated, organized, and able to work independently. Valid driver’s license and insurance as required by state(s) where employed.
