DUTIES:
VA St. Louis Healthcare System is recruiting for a full time board certified physician with advanced clinical and primary care or geriatrics experience as well as demonstrated skills in management of patients with complex chronic illness in a predominantly geriatric population. Home and Community Based Care (H&CBC) provides a wide array of services in both institutional and non-institutional based care. These include an active and expanding Home Based Primary Care (HBPC), and Medical Foster Home Care.

Home visits are required to patients needing medical care and management in the home setting. Opportunity to participate in the growth and expansion ! of various H&CBC services and provide primary care in the home setting through an interdisciplinary Team approach. Preceptorship, supervision of and collaboration with various physician extenders is required. Academic opportunities to train Geriatrics Fellows in home care are available.

Incumbent would be clinically privileged in Internal Medicine Family (and Geriatrics if qualified), commensurate with qualifications. This position will have clinical responsibility for the H&CBC services as a Team Physician for HBPC, and may provide additional responsibilities in the Community Living Center, a skilled nursing and rehabilitation Long Term Care facility in the Medical Center. This position reports to the Medical Director, H&CBC which is organizationally aligned under the Extended Care and Rehabilitation Service (EC&RS).

The incumbent will function as an active member of the Medical & Dental staff. The VA St. Louis Healthcare System is JCAHO accredited, and i! ncludes a CARF accredited Comprehensive Medical Rehabilitation in-patie! nt unit in a skilled nursing and rehabilitation Long Term Care facility, and is a Polytrauma Network Site. The Medical Center is highly affiliated with several academic institutions, including Washington University and St. Louis University.

The Physician also functions as an integral member of the medical team within EC&RS and the Medical Center by the promotion and maintenance of quality, patient-centered, health services through a seamless continuum of care at both Divisions of the Medical Center. On-call responsibilities will be proportionately shared with other full-time EC&RS physicians. Opportunities for active involvement in academic teaching are available, and research is encouraged.

QUALIFICATIONS REQUIRED:

egree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine.

Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.

PHYSICAL REQUIREMNTS: the following tasks which may be performed intermittently: lifting of up to 10 lbs., sitting (4 hours),standing (2 hours), walking (2 hours), bending/stooping (1 hour), twisting (1/2 hour), simple grasping (4 hours), fine manipulation (6 hours), reaching above the shoulder (1/2 hour), and operating machinery (1/2 hour).
